alpha fade based on distance from camera

I know this has been posted about before, but I can't seem to find a good answer (specifically a screenshot of what the shader forge would look like). 

I'd like to just fade the alpha based on distance from the camera. How do I do that?

Plug in Clamp01( 5 - depth ) into Alpha:

oops, I just read the instructions on the front page, I will repost in the forums
the UI of this UserEcho thing is much better than the forums though, I find it difficult to search the Unity forums for specific posts
