That's probably your quality settings. Open them and up the pixel light count :)
You have to be more elaborate than that.
On which platform? Did you check all the render platforms that you need? Using what setting? Did you use Custom lighting or one of the built in ones?
The Unity 5 features are still in beta, since Unity 5 itself is in beta! We'll see when it works out, but I don't have an ETA at the moment
Try flipping it based on the projection parameter node's sign output

I've now added a warning in the SF console when this happens, as of 1.03 :)
Some of the nodes share the same internal name, for some reason.

Which version are you using, and which version did you start creating the shader in?

To fix it, go to settings and enable variable name & precision display, and then make sure they are all unique.
This doesn't seem to happen anymore
As of 1.03, you can now hold Ctrl to temporarily toggle hierarchal node movement
